If a student is required to take medication during school hours a parent or guardian must bring it to the School Nurse. All prescription medications must be in their original bottles with student’s name, dosage, etc. typed on and must be accompanied by the appropriate forms signed by a doctor. The forms may be obtained from the School Nurse. Students are not to carry any medication, prescription or over-the-counter. Students who have asthma and who use an inhaler should also have the appropriate form completed by the doctor and an inhaler in school. It is very important that students return three completed emergency cards and a school health form. Please revise the form any time information changes. It is critical that we have a current telephone number on hand to reach parents/guardians.
